Role: Licensed Speech Language Pathologist providing personalized care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model. Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.Provide one-on-one therapy sessions in private residences and senior living settings.Document patient progress accurately in electronic medical records, adhering to regulatory standards.Collaborate with an interdisciplinary team including PTs, OTs, and other specialists.Educate patients, caregivers, and staff on safety and aging-in-place strategies.Utilize telehealth and virtual care options to enhance access and flexibility. Qualifications & Skills Valid licensure or eligibility in Speech Language Pathology.Degree from an accredited program and current ASHA CCC certification.Proficiency with basic computer and EMR systems.CPR certification.Physical ability to perform in-home care, including lifting, repositioning, and assisting patients.
