Speech Language Pathologist providing in-home, senior living, and virtual care to older adults.
Responsibilities
- Conduct evaluations
- Develop individualized treatment plans
- Deliver evidence-based therapy to support patient wellness, mobility, and independence
- Document care in EMR
- Collaborate with interdisciplinary teams
- Educate patients and caregivers
- Utilize telehealth when appropriate
Qualifications
- Valid SLP license
- Degree from an accredited program
- EMR proficiency
- CPR certification
- ASHA CCC
Physical Requirements
- Ability to perform moderate physical effort
- Assist with patient transfers
- Reposition patients
- Drive to patient locations
