Speech Language Pathologist responsible for providing personalized, one-on-one speech therapy to older adults in private homes and senior living communities.

Responsibilities

• Conduct evaluations and develop individualized care plans.
• Deliver evidence-based therapy to improve mobility, independence, and quality of life.
• Document patient progress in electronic records.
• Collaborate with interdisciplinary teams.
• Educate patients and caregivers.
• Utilize telehealth when appropriate.

Qualifications

• Valid SLP license or eligibility.
• Degree from an accredited program.
• Proficiency in computer and EMR systems.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and driving to care sites.
