About the role
Speech Language Pathologist responsible for providing personalized, one-on-one speech therapy to older adults in private homes and senior living communities.
Responsibilities
- Conduct evaluations and develop individualized care plans.
- Deliver evidence-based therapy to improve mobility, independence, and quality of life.
- Document patient progress in electronic records.
- Collaborate with interdisciplinary teams.
- Educate patients and caregivers.
- Utilize telehealth when appropriate.
Qualifications
- Valid SLP license or eligibility.
- Degree from an accredited program.
- Proficiency in computer and EMR systems.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and driving to care sites.
