Speech Language Pathologist

Garden Spring Rehab and Care Center is seeking a PRN Speech Language Pathologist to join our team of health professionals in providing excellent health care to our residents. Responsibilities

• Assessment of referred patients requiring rehabilitation services.
• Providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of a physician.
• Setting realistic and achievable goals for patients.
• Documenting and recording the patient’s condition.
• Educating patients and families in appropriate therapy methods.
• Completing evaluations, weekly progress notes, recertification, and discharge documentation.
• Collaborating with all disciplines to provide the best patient-centered treatment and care.
• Performing all other duties assigned by the Director of Rehabilitation.
• Ensuring compliance with confidentiality, HIPAA, and Federal Residents' Rights.

Requirements

• Graduation from an accredited Speech Therapist college or university.
• Valid professional license and registration for the state of Pennsylvania.
• Experience with long-term care and/or geriatric care is preferred but not required.
