About the role
Speech Language Pathologist (SLP) providing personalized care to older adults in private residences and senior living communities.
Responsibilities
- Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
- Provide in-home, community, and virtual therapy sessions, documenting accurately in EMR systems.
- Collaborate with an interdisciplinary team including PTs, OTs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
Qualifications
- Valid state license or eligibility, and current ASHA CCC certification.
- Proficiency in computer and EMR systems, CPR certification.
- Ability to perform physical tasks such as lifting, repositioning patients, and assisting with transfers.
