Speech Language Pathologist

About the role

We are seeking a committed and patient-focused Part-time Speech Language Pathologist at Accela Rehab and Care Center of Somerton in Philadelphia, PA. Our facility offers a friendly, home-like atmosphere where warm and caring nurses and therapists guide residents on their road to recovery. We are dedicated to helping residents achieve their highest level of function and independence through a patient-centered approach, supported by specialty practitioners, therapists, nurses, and staff.

Responsibilities

• Assessment of referred patients requiring rehabilitation services.
• Providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of a Physician.
• Setting realistic and achievable goals for patients.
• Documenting and recording the patients’ condition.
• Educating patients and families on appropriate therapy methods.
• Completing evaluations, weekly progress notes, recertification, and discharge documentation.
• Collaborating with all disciplines to provide the best patient-centered treatment and care.
• Performing all other duties assigned by the Director of Rehabilitation.
• Ensuring compliance with confidentiality, HIPAA, and Federal Residents Rights.

Requirements

• Graduation from an accredited Speech Therapist college or university.
• Valid professional license and registration for the state of NJ.
• Experience with long-term care and/or geriatric care is preferred but not required.
