Key Responsibilities
- Engage directly with patients to coordinate pharmacy services through the health system's specialty and retail pharmacy.
- Act as a bridge between patients, providers, and the pharmacy, focusing on improving prescription capture, providing follow-up counseling, and ensuring smooth communication.
- Process prior authorizations, assist with financial aid, update clinical data, and support administrative tasks.
- Collaborate closely with the CPS Patient Care Services team and work under the direct supervision of a pharmacist.
- Initiate and lead ongoing work in clinics, ensuring that the patient/CPS engagement is appropriate and consistent with their preferences.
- Ensure every patient who can benefit from specialty pharmacy services is identified and met in clinic upon arrival.
- Explain the benefits of our specialty pharmacy, without impeding patient choice of pharmacy, and achieve patient participation.
- Successfully enroll patients into the program.
- Perform all benefits investigation, prior authorization, and financial assistance work needed to ensure patients have access to the medication they need.
- Oversee services provided to enrolled patients - ensuring that everything from order entry to final verification is completed quickly, accurately, and in accordance with patients' needs.
- Provide direct support to enrolled patients to ensure they are supported throughout their therapy regimen and achieve their intended outcome.
- Conduct outbound phone calls for patient support, adherence checks, and refill reminders.
- Accept inbound calls from patients, as needed, and share in on-call coverage to ensure 24/7 patient support.
- Appropriately triage any clinical questions to an appropriate clinician (pharmacist, nurse, or provider).
Qualifications
- High school diploma or equivalent.
- Active Illinois Pharmacy Technician license in good standing.
- PTCB certification or ability to acquire within 24 months of hire.
- College degree preferred.
- Experience in a health system specialty pharmacy as an in-clinic liaison.
- Experience working with various disease states.
Pay & Benefits

The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment.
