Specialty Pharmacist

The Opportunity We are looking for a Specialty Pharmacist to play an important role at AndHealth. This is a full-time position located at a Community Health Center in Illinois.

Responsibilities

• Deliver direct care, therapeutic recommendations, and counseling for patients with chronic conditions via the Patient Management Program, protocols, or collaborative practice agreements.
• Review patient profiles for interactions, allergies, or duplications, and recommend evidence-based medication or dosage adjustments.
• Partner with Patient Engagement Specialists to secure patient access by managing payor formularies, prior authorizations, and appeals.
• Maintain strict documentation for URAC and ACHC accreditation and actively participate in Quality Management programs and improvement projects.
