Gaudenzia's Mission is to promote long-term recovery by providing comprehensive treatment and support to people with substance use disorder, co-occurring mental health disorders, and related conditions, regardless of individual circumstances.

Responsibilities

• Design, develop, implement, supervise, and provide specialized training and services to clients in the program
• Handle an independent caseload or assist other personnel in counseling clients to aid them in achieving adjustments to specific problems or situations of a difficult or complex nature
• Conduct intake interview and orientation to care coordination with client prior to discharge from residential level of care
• Liaise with clinical staff for review and verification of recommended services, referral to clinically recommended level of care, and collateral contact information
• Complete Recovery Plan in coordination with client and primary clinical counselor
• Identify health care and recovery support needs with client and initiate referrals
• Assist clients in transitioning to the next level of care by removing barriers, providing reminders, and conducting outreach when appropriate
• Develop and maintain relationships with services and programs, including but not limited to treatment, health care, employment, legal, housing, child welfare, social services, and peer-support providers
• Establish and maintain care coordination services and contact schedule
• Maintain client records and complete all other required documentation
• Assist residents in education around program offerings, supports, and ways to access them
• Reduce barriers to care by addressing logistical, financial, cultural, and systemic challenges
• Ensure client medical, psychiatric, and outpatient needs are met and referrals are responded to in a timely manner
• Educate on day-to-day symptom management, medication management, and comorbid conditions
• Coordinate timely access to appropriate RVWV and community providers based on clinical recommendations and resident disposition throughout treatment
• Ensure resident community appointments are maintained
• Maintain engagement during transitions
• Link patients to recovery supports and community resources
• Conduct post-discharge follow-up
• Assist in reducing readmissions by providing interventions based on post-discharge follow-up responses from residents

Documentation, Quality & Compliance

• Maintain accurate EHR documentation
• Track access, engagement, and outcomes
• Comply with HIPAA and 42 CFR Part 2
• Participate in performance improvement and satisfaction initiatives

Requirements

Education and Training

• A minimum of an Associate Degree or full certification as an Addictions Counselor
• Associate Degree: Five (5) years of experience
• Bachelor's Degree: Three (3) years of experience
• Master's Degree: Two (2) years of experience
• Certified Addictions Counselor (CAC): Five (5) years of experience

Skills, Knowledge, And Abilities

• Specialized training and experience in working with specific populations
• Knowledge of the basic principles, practices, and procedures involved in an alcohol and drug program
• Knowledge of drug and alcohol-abusing subculture
• Knowledge of the physical and emotional characteristics and manifestations of alcohol and drug dependency
• Knowledge of individual and group behavior and effective ways of working with them
• Knowledge of the continuum of care as it relates to drug and alcohol treatment
• Knowledge of confidentiality and all other applicable federal and state regulations in reference to specialty area, and ability to ensure program compliance
• Ability to perform needs assessment and develop appropriate programming
• Ability to work effectively with people and aid them in adjusting to specific problems
• Ability to establish and maintain effective working relationships with clients, staff, outside agencies, institutions, and the general public
• Ability to design, implement, and train others pertaining to the specialty area
• Ability to communicate effectively and clearly, both verbally and in writing
• Ability to present effective seminars, workshops, and presentations to clients, staff, and the community
• Ability to work independently and exercise independent judgment
