About the role
The Special Teams Home Health Aide position at HCR Home Care requires employees to be available during their designated shift to support the on-call team, congregate team, Private Pay Team, or the Certified Home Health Agency (CHHA) team. Minimum availability is 16 hours per month on weekends.
Responsibilities
- Be available during designated shifts and accept all assignments made during those times.
- Available for cases involving cats, dogs, and smoking, and willing to work with various types of clients.
- Arrival/Departure times are mandatory within specified hours, with deviations arranged with the Aide Supervisor and Aide Services Coordinator.
- Receive specialized training in topics such as NHTDW, Companion Care, Memory Care, and Palliative Care.
- Exercise professionalism, confidentiality, compassion, and empathy when caring for clients.
- Availability for on-call Aides includes 24-hour cases and follows the 24-hour case policy.
- Adhere to essential functions as required of an HHA, including providing health-related services, assisting with personal hygiene, and reporting changes in client's condition.
- Complete and submit necessary documentation, attend in-service programs, and ensure client safety.
- Provide one’s own transportation and arrive promptly to all assigned HCR work locations.
- Communicate effectively with clients and interdisciplinary teams, and be able to speak and write in English.
- Other duties as assigned.
Qualifications and Requirements
- High school diploma required.
- HHA certification in good standing, according to NYS regulations.
- Mature and able to deal effectively with the job and its demands.
- Respectful of client’s rights and treat them with dignity and respect.
- Have a sympathetic attitude towards the care of the sick and disabled.
- Availability requirements are 40 hours per week for Full-time.
- Active/valid driver’s license and reliable transportation.
- Working phone for daily communication with the Aide Services Coordinator.
Education Requirements
A high school diploma is required.
Physical Requirements
- Stand, sit, hear, walk, talk, stoop, or kneel.
- Repetitive motion.
Work Environment
The Special Teams HHA is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work: Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
