Spanish Speaking Social Worker (Master of Social Work)

Job Summary

At Brighton/Suncrest Hospice, we aim to elevate hospice care in your area through enhanced staffing and clinical visits. We are CHAP certified and seek committed professionals to join our team.

Responsibilities

• Assesses psychosocial status and social/emotional factors related to patients’ conditions and needs.
• Counsels individuals and families, assisting with goal setting and planning interventions.
• Updates case records and maintains communication with other healthcare providers and clients.
• Maintains client care records and reports client responses to treatment and condition changes.
• Attends and participates in client care conferences, staff meetings, and agency committee meetings.

Requirements

• Master’s degree in Social Work required.
• Thorough knowledge of social work principles and methods.
• Knowledge of individuals and group behavior, and skills in working effectively with diverse populations.
• Ability to exercise independent professional judgment in complex situations.
• Ability to establish and maintain effective working relationships with clients, families, staff, and the public.
• Effective communication skills, both oral and written.

Pay and Schedule

Salary range: $75,000 - $85,000.
