Health Plus Management (HPM) is a Physician Support Organization and market leader in clinical practice management in the physical medicine and rehabilitation market with 50+ locations serving Long Island, NYC, Westchester, Upstate NY, NJ and Connecticut. HPM provides independent physician practices specializing in musculoskeletal care with professional business development, marketing and back-office support, enabling clients to start and/or grow their private practice while maximizing clinical service and patient care.

About the role

The Patient Care Coordinator will serve as the liaison between our physicians and patients, welcoming patients and providing exceptional customer service throughout their visits. In this role, you will educate patients on their specific treatment plans and respond to all inquiries with compassion.

Responsibilities

  • Provide outstanding customer service to every patient
  • Coordinate patient visits to ensure proper progress of treatment plans
  • Educate patients on treatment plans, practice policies, and financial responsibility
  • Schedule treatment plans with patients in alignment with physicians' recommendations
  • Track referrals and treatment plans with patients
  • Assist patients in setting goals and identifying issues when goals are not met
  • Help patients complete necessary paperwork
  • Respond to patient inquiries in a compassionate and timely manner
  • Process medical insurance information, verify patient eligibility, and address insurance-related questions (liaise with Billing department as needed)
  • Ensure compliance of treatment plans created by physicians
  • Maintain confidentiality of personal information (HIPAA compliant)
  • Ensure all patient information (demographics, insurance, etc.) is accurately recorded in the system for follow-up and reporting
  • Provide advocacy, information, and referral services to patients
  • Act as a backup to other team members as needed
  • Perform other duties as assigned

Requirements

  • 1+ years of relevant medical experience, preferred
  • Knowledge of medical terminology and procedures, with the ability to converse confidently with patients
  • Experience with EMR systems
  • Strong computer skills, including proficiency in Microsoft Word and Excel

  • Prolonged periods of sitting at a desk and working on a computer

Schedule

Full-Time, Monday–Friday, 9:00 AM–5:00 PM

Pay

$20–$22 per hour
