Social Worker (SW), PRN
About the role
As a Social Worker at North Alabama Medical Center, you will play a crucial role in helping patients cope with various social, emotional, economic, and environmental issues. You will interview and assess patients and their families, determine necessary services, and assist with discharge planning.
Responsibilities
- Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
- Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
- Participates in case reviews to evaluate case management and progress.
- Consults with healthcare team members to promote, monitor, and evaluate compliance with patient’s treatment plan.
- Assists with discharge planning and processes.
- Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
- Supervises and/or trains new staff, students, and interns.
- Provides individual, family and group therapy.
Requirements
A Bachelor's degree is required. A Board Certified Social Worker license in the state of Alabama (LBSW, LCSW) must be obtained within six months of hire.
Skills
Critical thinking skills, decisive judgment, and the ability to work with minimal supervision are essential. Must be able to work in a stressful environment and take appropriate action.
