Job Duties
- Counsels patients and families on bio-psychosocial issues.
- Coordinates and implements complex discharge plans.
- Communicates with the multidisciplinary team to evaluate and respond to patient and caregiver needs.
- Maintains patient records and documents critical information.
- Provides referrals and helps identify helpful resources for patients and families.
Knowledge, Skills and Abilities (KSAs)
- Proficient with Windows-style applications and various software packages specific to role.
- Effective verbal and written communication skills.
- Working knowledge of common disease entities and disorders, medical terminology, and the psychosocial and economic effects of illness and/or trauma upon the patient and their support system.
- Knowledge of health care systems and public programs such as Medicare, Medicaid, Medicare Advantage, and commercial insurance plans.
- Strong interpersonal skills necessary to deal effectively and courteously with patients, families, multidisciplinary team members, departmental peers, and community agency personnel.
Education and Certification
- Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE).
- Certified Social Worker (CSW) as verified by the LABSWE OR Registered Social Worker (RSW) certificate.
- Possesses a License as a Master Social Worker (LMSW) as verified by the LABSWE preferred or must schedule and pass LMSW exam within 6 months of employment.
Physical and Environmental Demands
- Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
- Sit and stand for prolonged periods of time.
- Travel throughout and between facilities.
- Work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability).
