Hours: 8:00am - 4:30pm
Schedule: 5 Shifts per week. Weekends required.
General Duties
The individual in this position is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. The individual in this position has overall responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and interventions to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services.
Department Specific Duties
- Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
- Assesses for potential abuse, mental health and substance abuse needs, and formulates appropriate recommendations for intervention
- Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
- Assists patients with resolving complex social issues that may interfere with treatment, or delay discharge, such as homelessness, lack of access to medical care, etc.
- Education provided to physicians, patients, families and caregivers
- Participates in planning the social work component and works in collaboration with case managers to develop a discharge plan that meets patients’ after-care needs.
- Collaborate with community providers to develop educational resources appropriate for staff and patients/patient representatives
- Provides patients and their families with brief counseling as needed
- Assists in the orientation and training process for new hires
Qualifications
- Master’s Degree in Social Work (MSW)
- Must be certified with Riverside County to write 5150 within 90 days of employment.
- If registered as an ACSW, must maintain an active status per state guidelines
- Apply for CA Children Services (CCS) paneling upon completion of 90-day orientation period
- Required qualifications include Master in Social Work (MSW). Must be currently licensed, certified, or registered to practice profession as required by law or regulation in state of practice or policy.
- Preferred: Accredited Case Manager (ACM). Minimum of two years acute hospital experience preferred.
- Demonstrated organizational skills, excellent verbal and written communication skills, ability to lead and coordinate activities of a diverse group of people in a fast paced environment, critical thinking and problem solving skills and computer literacy.
Training Requirements
Must complete and demonstrate competency in using the Tenet Case Management documentation system within 30 days of hire. Attendance at hospital and department orientation is required. Department orientation includes review and instruction regarding Tenet Case Management and Compliance policies, Transition Management, and other topics specific to case management.
