Social Worker | Regional Float

About the role

Join the inpatient care management team as a regional float social worker. In this role, you will support multiple hospitals by providing clinical social work services to patients and families during challenging times. You will collaborate with interdisciplinary teams including nurses, therapists, physicians, and dieticians, and manage a designated caseload to plan care, monitor patient stay, and promote community resources.

Responsibilities

• Assessing patient needs

• Assisting with transitional care

Qualifications

• Current LSW license in Minnesota
• Bachelor’s or Master’s degree in Social Work
• At least 2 years of clinical experience
• Valid driver’s license
• ACMA Case Manager‑SW Certification (to be obtained within five years)
