Float Social Worker

Job Summary: Gardner Heights Health Care Center, a 110-bed skilled nursing community located in Shelton, Connecticut, is seeking a compassionate and skilled Float Social Worker to join our dynamic team of three Shelton skilled nursing communities (Gardner Heights, Shelton Lakes and Hewitt). As a Float Social Worker, you will play a vital role in implementing and documenting plans of care related to residents' social/emotional needs, collaborating with multidisciplinary teams, and ensuring excellent resident care outcomes. This role is in addition to the three full time Social Workers stationed at each location. This is a full time benefit eligible position scheduled at 32 hours per week. Responsibilities: Implement and document plans of care related to residents' social/emotional needs in the electronic medical recordCollaborate with nursing, MDS, and rehabilitation departments to facilitate discharge plans for residentsInform and educate staff, residents, families, and responsible parties of residents' personal and property rights, as well as their rights related to health servicesMaintain updated resources for residents and families regarding community resources and servicesInvestigate reported concerns, assist in resolving them, and ensure appropriate follow-up and documentationProvide timely referrals to appropriate agencies for assistance to residents and families Requirements: Bachelor's or Master's Degree from an accredited program in Social Work or related fieldPrevious experience in a long-term care facility, preferably with knowledge of current Requirements of Participation regulationsKnowledge of admissions and discharge procedures, as well as residents' rights and documentation requirementsAttention to detail, good follow-through skills, and ability to prioritize multiple tasksInnovative and able to find creative ways to serve the variety of resident needsMust have a vehicle and a valid driver's licenseDemonstrates cooperation in assisting others and ability to work as a teamPatience, tact, enthusiasm, and a positive attitude toward residents and their support networksAbility to remain calm in stressful situationsAbility to communicate with residents, staff, and family members in a professional manner What We Offer: Attractive benefit package for employees of 30 hours or greater, including:Scholarships and career growth opportunities4 Weeks Paid Time Off7 Paid HolidaysHealth Insurance BenefitsShort Term & Long Term DisabilityCall-a-Doc / 24-7 MD telephone serviceEmployee Assistance ProgramLife Insurance401K Retirement ProgramLongevity Credit How to Apply: If you are a motivated and compassionate professional looking for a rewarding career opportunity, please apply to this Float Social Worker position at Gardner Heights Health Care Center.
