About the Role

Support Medicare patients in underserved communities as a Medical Social Worker Float, conducting psychosocial assessments, developing care plans, and addressing social determinants of health across multiple clinics.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide brief counseling and crisis intervention.
• Address social determinants of health to improve patient outcomes.
• Coordinate community resources, benefits, and care transitions.
