About the role
Elderwood at Burlington is a 150-bed skilled nursing and subacute rehabilitation facility in Burlington, Vermont. We provide comprehensive skilled nursing care, long-term care, and rehabilitation services including orthopedic rehabilitation, physical therapy, and speech therapy. Specialty programs include palliative care, respite care, and wound care.
Responsibilities
- Welcome and support residents and families during the admission process
- Assess psychosocial, emotional, and support needs of residents
- Participate in care planning and interdisciplinary team meetings
- Aid with discharge planning and transitions of care
- Complete required documentation, including admission and discharge forms (including SCREEN when applicable)
- Provide counseling, emotional support, and resource referrals to residents and families
- Advocate for resident rights, dignity, and quality of life
- Collaborate with nursing, therapy, and clinical teams to support resident needs
Qualifications
- Minimum of a bachelor’s degree in social work or related field from an accredited institution
- One year of supervised social work experience in a healthcare setting working directly with individuals
- Valid Drivers License and clean driving record required
- Work experience in the admission or discharge area of social work very desirable
- Desire and ability to work with the elderly and to cooperate with other staff members necessary
