Responsibilities

The Social Worker strives to:

• Promote patient and family wellness
• Improve care outcomes
• Ensure access to appropriate hospital and community resources
• Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments

Requirements

• Master's degree from an accredited social work program Required
• 2 years of experience in an acute or post-acute setting Required

Qualifications

• LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia Preferred
