Social Worker (Program Coordinator) Caregiver Support

About the role

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Responsibilities

  • Develops and implements the Caregiver Support Program specific to area of assignment and ensures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives
  • Completes initial, interim, and annual in-home visits for patients enrolled in the Caregiver Support Program who reside in VA ECHCS' catchment area and includes rural areas locations - travel is necessary for half of the tour
  • Administers information and analytical systems to evaluate and enhance the quality of service provided to Caregivers - families - and Veterans
  • Develops and implements information systems to track service activities including visits - referrals - Caregiver demographics - equipment utilization - and Caregiver and Veteran satisfaction
  • Develops - implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services
  • Participates in direct Caregiver and Veteran contacts to identify and assess Caregiver stress and the development and application of directed clinical treatment interventions
  • Advises and collaborates with interdisciplinary teams throughout the medical center on Caregiver issues
  • Creates educational tools - develops programs - and implements training focused on specific Caregiver needs/issues
  • Provides education and in-service training to employees - Veterans - families - and Caregivers
  • Provides ongoing consultation and support to colleagues regarding Caregiver support
  • Distributes educational materials provided by VA - local - state - and national Caregiver organizations
  • Conducts Caregiver and family clinical interventions to facilitate the identification of Caregiver stress - psychiatric illness - emotional distress - and social needs as treatment issues
  • Serves as the major consultant for information on negotiating the complex processes Caregivers - Veterans - and families encounter when interacting with government and community agencies
  • Stays abreast of current federal and state legislation directed at assisting Caregivers to continue to provide caregiving in an environment that is safe for both themselves and the Veteran
  • Maintains a knowledge base of current theory and treatment modalities by reading professional journals - attending conferences - clinic seminars and teleconferences - and materials distributed through VA and private sector sources
  • Collaborates with the patient aligned care team - mental health clinicians - and specialty care providers to provide Veteran and staff education on Caregiver issues
  • Performs a wide range of services and provides information to state - local - and private agencies
  • Uses sound judgment in the interpretation of trends and patterns - the application guidelines - and program enhancements
  • Performs program analysis - development - review - research - design - and contact work as possible with various agencies (public - DOD - state - etc.)
  • Works closely with the patient-aligned care team to ensure the successful implementation of the Caregiver Support Program
  • Coordinates with local facilities to ensure seamless integration of the Caregiver Support Program
  • Ensures compliance with VA policies and procedures related to the Caregiver Support Program
  • Participates in the development and refinement of clinical guidelines and protocols for the Caregiver Support Program
  • Provides ongoing support and training to Caregivers and their families to improve their caregiving skills and reduce stress
  • Facilitates the establishment of Caregiver support groups and educational programs
  • Assists in the recruitment and retention of qualified Caregiver Support Program staff
  • Monitors and evaluates the effectiveness of the Caregiver Support Program
  • Collaborates with external partners and stakeholders to enhance the impact of the Caregiver Support Program
  • Provides input on the development of new programs and initiatives related to Caregiver support
  • Works with interdisciplinary teams to address the unique needs of Veterans and their families
  • Participates in the development and implementation of policies and procedures related to the Caregiver Support Program
  • Provides leadership and mentorship to Caregiver Support Program staff
  • Participates in the evaluation of the Caregiver Support Program and makes recommendations for improvement
  • Works with the patient-aligned care team to ensure the successful implementation of the Caregiver Support Program
  • Collaborates with local facilities to ensure seamless integration of the Caregiver Support Program
  • Ensures compliance with VA policies and procedures related to the Caregiver Support Program
