Social Worker

Serenity Hospice is seeking a PRN Social Worker for their Salem team. The position offers flexible hours, a supportive culture, and comprehensive benefits.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
• Assists in providing an assessment in the patient’s identified residence and assists when necessary, ensuring another plan is required.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Affords assistance to team members as needed to maintain consistent high levels of service.
• Counsels patient and family/caregivers and staff as needed regarding stress and other identified coping difficulties.
• Accurately and timely documents patient services to reflect the plan of care.
• Assists with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver.
• Obtains resources from community social, health, pharmaceutical, and welfare agencies to assist in meeting patient/family needs.
• Serves as a liaison between patients and families/caregivers and community agencies.

Qualifications

• Oregon: Master’s Degree in Social Work required.
• Current State Licensure in Social Work (Oregon: Licensed not required).
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance is required.
