Role Overview
The Social Worker provides medical social services under physician and interdisciplinary team guidance to support patients' emotional, social, and environmental needs related to their health.
Responsibilities
- Assist patients, families, and healthcare providers in understanding social factors affecting health
- Contribute to developing comprehensive Care Plans
- Educate team members on community resources
Qualifications & Skills
- Master's Degree in Social Work from an accredited program
- Valid driver's license, vehicle insurance, and reliable transportation
- Preferred: over a year of healthcare social work experience
- Preferred: current CPR certification
- Preferred: licensure as a Social Worker in their state
