Social Worker
About the role
The social worker serves as a member of an interdisciplinary treatment team contributing to overall primary healthcare services to Veterans assigned to a primary care team. The social worker provides clinical counseling, psychosocial assessment, and case management services to eligible veterans and their family members to meet biological, psychological, social, and environmental needs. The social worker adheres to the principles of the Patient Aligned Care Team (PACT) Model.
Responsibilities
- Interviews clients and conducts social work comprehensive assessments or psychosocial assessments
- Treatment planning/goal setting
- Referral to service providers
- Resource development
- Advocacy
- Crisis intervention
- Supportive counseling
- Documentation and administrative functions
Requirements
Open to current, permanent employees of the Chillicothe VA Medical Center and Community Based Outpatient Care (CBOC) locations and all US Citizens.
Qualifications
- Master's degree in social work from a CSWE-accredited school
- Licensed or certified by a state to independently practice social work at the master's degree level
- One year of post-MSW experience equivalent to the GS-9 grade level in health care or other social work-related settings, or a doctoral degree in social work may be substituted for the required one year of professional social work experience in a clinical setting

Skills
Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. Ability to utilize a Veteran-centric approach when providing interventions and counseling. Ability to assess psychosocial functioning and needs of Veterans and their family members. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies.

Acceptable Forms of Identification
Full-time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
