SOCIAL WORKER MSW FMLH, FH - CARE TEAM MANAGEMENT OP

Location: US:WI:MILWAUKEE at Froedtert Hospital facility. On‑site. FTE: 1.000000.

Responsibilities

• Coordinating the discharge plan of care for all patients during their hospital admission.
• Coordinating the discharge plan starting on admission and through discharge follow‑up.
• Providing biopsychosocial assessments.
• Providing updates that address the patient’s evolving discharge care needs to timely move them to the appropriate next level of care.
• Establishing a plan of care that provides maximum benefit to patients and their families in collaboration with the multidisciplinary team in Care Coordination Rounds (CCRs) seven days a week.

Requirements

• Minimum of two years experience in an acute care setting (preferred).
• Casework in medical social work (preferred).
• Master's degree in Social Work (required).

Skills

• Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients and families.
• Analytical skills necessary to assess patients’ needs, develop associated discharge planning, and develop solutions to problems concerning patients’ mental and physical well‑being.

Licensure

• Current State of Wisconsin license as a social worker (required).
