Social Worker MSW Care Mgmt - FT - HPW

About the role

The position involves assessing, intervening, and planning appropriate discharge for patients based on their clinical, social, and functional needs. It also requires collaborating with the care management team to address complex social and financial barriers to timely discharge.

Responsibilities

• Documents clinical record, comprehensive assessment of patient needs, clinical interventions, and appropriate referrals.
• Affords culturally competent care and identifies barriers to timely discharge, implementing intervention plans.
• Collaborates with the health care team to identify and address social and financial barriers to appropriate transitions.
• Performs educational sessions for the health care team on abuse, neglect, and reporting.
• Advocates for appropriate services for patients with payors and providers of post-acute care, community services, and other services to optimize patient outcomes.
• Participates in staff development and performance improvement programs to enhance self-knowledge and awareness of bio-psycho-social issues.
• Attends and participates in regular departmental performance programs, staff meetings, and hospital-wide committees.
• Completes mental health and psych assessments when necessary.
• May perform follow-up services to measure clinical outcomes.

Requirements

• Master's degree in Social Work (MSW).
• One year experience in hospital case management or healthcare.
• Licensed Social Worker with 4 hours of Stroke-related continuing education contact hours annually if assigned to certain units.

Qualifications

• Excellent communication, conflict management, organizational, and planning skills.
• Knowledge of spreadsheet and word processing software, case management and patient information systems.
