Join Our Team as a Social Worker

Responsibilities
- Psychosocial evaluations
- Ongoing counseling of patients and families during their end-of-life journey
- Provide emotional support to patients and families when it's needed most
- Work in accordance with the plan of care
Requirements
- Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
- Licensure: Current state license as a social worker
- Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
- Reliable transportation
- Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively
Qualifications
- A heart to serve patients and families and a passion for providing the best possible care
Skills
- Strong interpersonal and communication skills
- Excellent problem-solving abilities
- Knowledge of hospice and palliative care principles
