Social Worker (MSW)

About the role

DaVita is seeking a Licensed Social Worker (LMSW) to support patients and their families at our Sioux Falls and Moccasin Creek Dialysis Clinics. The role involves providing clinical support, helping patients understand their rights and responsibilities, and building meaningful relationships.

Responsibilities

• Support patients and their families in understanding the physical, mental, emotional, and financial aspects of End Stage Renal Disease.
• Guide patients in managing the demands of their condition.
• Build and maintain long-term relationships with patients and their families.
• Advocate for patients' rights and needs.
• Collaborate with interdisciplinary teams to improve patient outcomes.

Requirements

• Licensed or registered in the state of practice as required by state regulations (LMSW or LCSW).
• Masters degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita specific applications within 60 days.

Pay

Compensation for the role will depend on a number of factors, including qualifications, skills, competencies, and experience.

Schedule

Split coverage between Sioux Falls and Moccasin Creek Dialysis Clinics, Monday - Friday schedule, full-time status with 30 hours per week.
