Responsibilities
- Conduct comprehensive biopsychosocial assessments to evaluate patient needs and identify any barriers to their overall functioning.
- Collaborate with the healthcare team and the patient to develop individualized care plans that address their unique needs, goals, and preferences.
- Connect patients and their families with community resources, mental health services, support groups, and other relevant programs.
- Educates patients and their families on available resources and coordinates and delivers patient education, referrals, and transitions of care.
- Accurately document and bill for services rendered in compliance with insurance and regulatory requirements.
- Identify and manage possible or actual patient/family crises, provide crisis intervention and emotional support, and create and facilitate effective plans for resolution.
- Utilizes motivational interviewing techniques to assist patients with reaching their health goals.
- Recognize and respect the unique values, beliefs, and cultural backgrounds of each patient, tailoring interventions and support to align with their individual needs and preferences.
- Build trust-based relationships with patients and their families by demonstrating empathy, confidentiality, and professionalism.
- Facilitates meetings with team members, provides training and education, develops standard work and oversees process implementation as appropriate.
- Serves as a consultative resource for team members, providers, and colleagues.
Requirements
- Master's Degree in Social Work from a program accredited by the Council on Social Work Education.
- Licensed as Licensed Clinical Social Worker (LCSW) in the state of North Carolina.
- Three (3) years of broad clinical experience in a healthcare setting for patients with complex medical conditions.
