Social Worker (MSW)

About the role

The Hospice Medical Social Worker is a key member of the interdisciplinary team, providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care.

Responsibilities

• Conduct social casework and assess families' financial situations, referring to resources as needed.
• Respond to referrals, participate in team conferences, and collaborate with external agencies.
• Facilitate communication about social resources and ensure use of the 4Ms (What Matters, Medications, Mentation, Mobility) for Age-Friendly Care.

Qualifications

• Master’s degree (MSW) from an accredited School of Social Work.
• Current Social Worker License per state requirements.
• Proficiency in clinical skills, communication, and documentation.
• Valid driver’s license with a good driving record.
• Availability during agency operating hours.

Preferred Experience & Certifications

• Advanced Palliative Hospice Social Worker Certification (APHSW-C).
• At least two years of hospice experience.
• Certifications such as CVA and EMR experience are advantageous.
