Role Overview: The Hospice Medical Social Worker is a key member of the interdisciplinary team, providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care. Key Responsibilities Conduct social casework and assess families' financial situations, referring to resources as needed.Respond to referrals, participate in team conferences, and collaborate with external agencies.Facilitate communication about social resources and ensure use of the 4Ms (What Matters, Medications, Mentation, Mobility) for Age-Friendly Care. Qualifications & Skills Master’s degree (MSW) from an accredited School of Social Work.Current Social Worker License per state requirements.Proficiency in clinical skills, communication, and documentation.Valid driver’s license with a good driving record.Availability during agency operating hours. Preferred Experience & Certifications Advanced Palliative Hospice Social Worker Certification (APHSW-C).At least two years of hospice experience.Certifications such as CVA and EMR experience are advantageous.
