Job Summary

Under minimal supervision and at the direction of the Case Management Director, the Inpatient Social Worker/Patient Counselor supports patients in addressing their bio-psychosocial needs, facilitating access to medical care and community resources to optimize their health outcomes. This role involves crisis intervention, short-term counseling, and transition planning, ensuring patients move smoothly along the continuum of care. The Social Worker/Patient Counselor also coordinates discharge planning by collaborating with patients, families, and healthcare teams.

Key Responsibilities

• Conduct bio-psychosocial assessments to determine patient needs.
• Provide crisis intervention and short-term counseling for patients and families.
• Assist patients in accessing medical care and community resources.
• Develop and execute discharge plans, collaborating with healthcare teams.
• Support patients transitioning across different levels of care.
• Ensure compliance with healthcare regulations and ethical standards.
• Maintain accurate documentation of patient interactions and treatment plans.

Required Qualifications

• Master's Degree in Social Work or Counseling Psychology.
• Minimum one year of experience as a Social Worker or Counselor.
