Responsibilities
- Attends various medical staff or other departmental and committee meetings as a required in representing patient interests and/or concerns.
- Administers patient assessment and writes evaluation report recommending treatment based on patient’s needs and function level.
- Involves family in the treatment and discharge planning process if agreed upon by the patient.
- Records patient progress notes in their respective charts.
- Active in the patient discharge planning process and assists in the patient’s discharge to home or other extended care facility placement.
Other Duties And Responsibilities
- If Master Social Worker may provide supervision to Bachelors Social Worker staff as needed.
- Collaborates with outside agencies to coordinate continued care.
- Selects and schedules treatment activities consistent with the changing needs of the patient.
- Coincides with other team members in the implementation of treatment plan; communicates recommendations to other disciplines.
- Keeps abreast of new regulations, procedures, equipment and methods.
- Evaluates patient goal and objective accomplishment and presents verbal and written criteria for termination of treatment.
Qualifications
- Knowledgeable of the Hospital’s computer system as it relates to the Social Services department and accessing patient related information.
- Excellent interpersonal and communication skills with the ability to deal with confidential information tactfully and diplomatically.
- Able to assist with the legal process as it relates to involuntary patients or guardianship.
Physical/Mental Requirements And Typical Working Conditions
- Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
- Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.
- Must be able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.
- Overall vision and hearing is necessary with or without assisted device(s).
- Frequent postural changes such as stooping, kneeling or crouching.
- Some exposure to blood borne pathogens and other potentially infectious material.
- Must follow MyMichigan Health bloodborne pathogen and TB testing as required.
- Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
- Overall dexterity is required including handling, reaching, grasping, fingering and feeling.
- May require repetition of these movements on a regular to frequent basis.
