Job Overview
As a Social Worker at Trinitas Regional Medical Center, you will provide critical frontline services to patients and families with conditions spanning the entire healthcare continuum. Operating as an essential member of the interdisciplinary team, this professional works in concert with doctors, nurses, and allied health staff to address and resolve the complex social, financial, and psychological problems related to a patient’s illness. The role focuses on delivering high-quality case management, facilitating supportive counseling, and implementing safe, timely discharge plans while sensitizing healthcare providers to the emotional aspects of patient care.
Qualifications
Master’s degree in Social Work (MSW) from an accredited School of Social Work.
Active New Jersey State Social Work Licensure: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
Strong verbal and written communication skills with a high proficiency in medical record documentation.
Proven ability to collaborate effectively within a fast-paced, multidisciplinary healthcare team.
Mandatory physical demands of the role, including frequent standing, walking, and typing inside an active acute care hospital setting.
Flexibility to work weekends and holidays on a rotational basis to meet departmental needs.
Preferred
Prior clinical social work or case management experience in a hospital or acute care setting is strongly preferred.
Bilingual fluency in English and Spanish.
Experience or training in palliative care coordination, goal-setting, and family care planning.

Essential Functions
Case Identification & Assessment: Proactively identifies, screens, and conducts comprehensive psychosocial assessments on cases from the general patient population.
Discharge Planning: Formulates and implements appropriate, timely, and safe discharge plans for all patients assigned to the caseload.
Counseling & Support: Provides frontline clinical counseling to patients and families regarding illness, coping, hospitalization, and transitional care needs.
Interdisciplinary Collaboration: Participates in interdisciplinary team rounds and maintains strong, collaborative partnerships with physicians, nursing staff, and allied health professionals.
Clinical Documentation: Provides accurate, clear, and timely documentation of all social work interventions directly inside the patient’s electronic medical record.
Palliative Care Coordination: Coordinates specialized social work services within the palliative care team; facilitates family meetings for goal-setting, manages clinical liaisons with community hospices, and documents targeted interventions.
Resource Navigation: Connects patients and families with essential community resources and social programs, executing consistent follow-up to track successful linkage.
Departmental & Data Support: Maintains detailed monthly statistics on social work interventions and provides active feedback to the Director regarding ongoing staff and patient needs.
Professional Development: Attends educational seminars and staff in-services, bringing significant resources back to present to the larger department.
Field Instruction: Provides professional field instruction and oversight for graduate or undergraduate social work students when available.
