Social Worker (MSW)

About the Role

The Hospice Medical Social Worker is a key member of the interdisciplinary team, providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care.

Responsibilities

• Conduct social casework and assess financial situations of patients and families
• Respond to referrals and participate in team and agency conferences
• Refer patients and families to community resources
• Interpret social needs to staff and ensure the use of the 4Ms (What Matters, Medications, Mentation, Mobility) for patient-centered care

Requirements

• Master’s degree in Social Work from an accredited program
• Current state license in Social Work
• Excellent clinical judgment and communication skills
• Proficiency in computer use
• Availability during agency hours

Preferred Qualifications

• Advanced certification in hospice social work
• Two years of hospice experience
• Familiarity with electronic medical records
