About the Role

The Hospice Medical Social Worker plays a crucial part in the interdisciplinary team, providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care.

Responsibilities

• Conduct social casework and assess family financial situations
• Facilitate referrals to community resources and support services
• Participate in interdisciplinary team and case conferences
• Interpret and connect patients with social resources
• Ensure the use of patient-centered care principles, including the 4Ms and Age-Friendly Care

Requirements

• Master’s in Social Work (MSW)
• Active state social work license
• Proficiency in clinical social work skills
• Strong communication and interpersonal abilities
• Ability to make sound clinical judgments

Preferred Qualifications

• Advanced certification in social work or related field
• Prior hospice or palliative care experience
• Familiarity with Electronic Medical Records (EMR) systems
