Job Summary and Qualifications

The Social Worker (MSW) at HCA Florida Largo Hospital is responsible for non-clinical, biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. Key responsibilities include crisis intervention, patient/family interventions, high-risk screening, emotional support, referrals for financial assistance or other needs, assisting in post-acute care placement, responding to consults, and maintaining knowledge of managed-care concepts.

• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual needs.
• Develop and implement appropriate discharge plans for high-risk patients.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers.
• Provide emotional support to patients and family members during complex and/or emotionally charged issues.
• Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals.
• Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.

What You Will Do in This Role

You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs. You will also maintain a thorough working knowledge of Medicare, Medicaid, and private payer regulations and processes.

Qualifications

• Master Degree in Social Work required.
• 1+ years experience or MSW Internship required.
• Licensed Clinical Social Worker preferred.
• Certification in Social Work preferred.
• Ability to deliver information in a clear, concise, and compelling manner.
• Ability to break down data into component parts to analyze and identify underlying issues.
