Social Worker MSW

About the role

Aspire Rural Health Systems is seeking a full-time Social Worker for their Med Surg department.

Responsibilities

• Serve as a liaison between patients, hospice staff and cancer center staff
• Assist patients and their families through in-home care needs, procedures, and in resolving problems and concerns
• Maintain patient satisfaction and contribute to the continuous improvement of hospital and health service operations
• Conduct assessments, offer supportive counseling, develop care plans, and connect patients with resources to support wellness and continuity of care through end of life

Requirements

• Master’s degree in Social Work (MSW) from an accredited program required
• A Social Worker License issued by the State of Michigan is required
• Minimum of 2 years of experience in healthcare social work preferred
• Interpersonal skills: supportive, active listener, effective communicator with patients, family members, medical and hospital staff, ability to be both a team member and a team developer
• Conciliation/facilitation skills; analytical; problem-solving skills
