Social Worker (MSW)
About the role
We are seeking hospice medical social workers who are passionate about helping patients receive the care they deserve and making a meaningful impact on patients and their families. In this role you will conduct psychosocial evaluations, provide ongoing counseling throughout the end‑of‑life journey, and deliver emotional support in accordance with each patient’s plan of care.
Responsibilities
- Conduct psychosocial evaluations of patients and families.
- Provide ongoing counseling and emotional support during the end‑of‑life journey.
- Work in accordance with each patient’s plan of care to address emotional and social needs.
- Collaborate with the supportive leadership team and interdisciplinary staff.
Qualifications
- Heartfelt commitment to serving patients and families.
- MS degree in Social Work from an accredited program approved by the Council of Social Work Education.
- Current state license as a social worker.
- 2+ years of clinical work experience, preferably in healthcare or hospice.
- Reliable transportation.
- Physical ability to sit, stand, bend, move intermittently, and lift at least 25 lb.
