Social Worker (Master of Social Work)

Job Summary

At Brighton/Suncrest Hospice, we aim to enhance hospice care in your area through increased staff-to-patient ratios, leading to more frequent clinical visits and reduced clinician caseloads. We are proud to be CHAP certified.

Responsibilities

• Assesses, diagnoses, and treats patient's mental and social conditions
• Counsels individuals and/or families
• Updates case records
• Provides direct social casework services including goal setting, counseling, and planning
• Advises on available community resources and their eligibility criteria
• Monitors and reports client's response to treatment and condition changes
• Maintains client care records and communicates with relevant personnel
• Attends and participates in client care conferences, staff meetings, and agency committee meetings

Requirements

• Thorough knowledge of social work principles and methods
• Knowledge of individuals and group behavior
• Ability to work independently and effectively with clients, families, staff, and external agencies
• Effective communication skills (both oral and written)
• Master’s degree in Social Work required

$75,000 - $85,000 annually
