Social Worker (Master of Social Work)

Job Summary

At Brighton/Suncrest Hospice, we aim to enhance hospice care in your area through increased staff-to-patient ratios and improved clinical visits. We are CHAP certified and seek committed professionals to provide exceptional care and support to patients and their families.

Responsibilities

• Assesses, diagnoses, and treats patient's mental and social conditions
• Counsels individuals and/or families
• Updates case records
• Assists with goal setting and planning
• Provides interventions
• Advises on available community resources and eligibility criteria
• Monitors client’s response to treatment and reports changes in condition
• Maintains client care records
• Attends and participates in client care conferences, staff meetings, and agency committee meetings

Requirements

• Thorough knowledge of social work principles and methods
• Knowledge of individuals and group behavior
• Ability to work independently in complex situations
• Effective communication skills (both oral and written)
• Master’s degree in Social Work required

Pay and Schedule

Salary range: $65,000 - $85,000
