Rockford Center is a 138-bed mental health facility in Newark, DE, offering inpatient and partial behavioral health programs for children, adolescents, adults, and older adults. As the only child inpatient behavioral health facility in Delaware, Rockford serves New Castle County and northern Maryland with an experienced senior management team.

About the role

The Social Worker supports patients and families throughout the continuum of care in a behavioral health hospital. This role involves conducting psychosocial assessments, developing individualized discharge plans, connecting patients with community resources, and advocating for recovery-focused services. The Social Worker collaborates with an interdisciplinary treatment team to address psychosocial, emotional, and environmental factors affecting treatment outcomes.

A two-week orientation (Monday–Friday, 8:30 AM–5:00 PM) is mandatory.

Responsibilities

  • Conduct psychosocial assessments by interviewing patients and family members to document history and identify treatment focus.
  • Participate in developing and implementing treatment plans, ensuring concise and realistic care plans are executed.
  • Provide individual, group, and family process-oriented therapies using various professional treatment modalities.
  • Document treatment in the medical record legibly and in a timely manner.
  • Develop and coordinate individualized discharge plans using treatment team input and chart information to determine aftercare needs.
  • Identify and assess family or community resources (e.g., group homes, mental health practitioners) to meet aftercare needs.
  • Attend and contribute to regular treatment team meetings, presenting patient cases, progress toward goals, and treatment recommendations.

Requirements

  • Master’s Degree in Social Work or related program from an accredited college (must be license-eligible).
  • LPCMH/LCSW preferred.
  • Previous experience in a behavioral health setting preferred.
  • Current CPR certification.
