Peachford Hospital is a 246-bed, acute care psychiatric hospital located in Dunwoody. We provide inpatient care for children, adolescents, adults, and seniors, in addition to partial hospitalization and other outpatient programs. Peachford has been providing compassionate care since 1973, serving nearly 10,000 patients every year.
About the role
Social Services Case Managers are part of an interdisciplinary treatment team charged with providing quality therapeutic and case management services to our patients from admissions to discharge. Case Managers assess patient needs using their own personal style while delivering superior quality care. This full-time, day-shift position is ideal for a Master’s-prepared professional who thrives in a collaborative environment and seeks the challenge and satisfaction of helping patients get the help they need.
Responsibilities
- Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus.
- Provide individual, group, and family solutions-focused interventions.
- Facilitate process and didactic groups.
- Develop an individualized discharge plan utilizing information obtained from the patient, treatment team, and family to determine patient needs.
- Identify family or community resources such as group homes, halfway houses, or treatment providers.
Requirements
- A license-eligible master’s degree in a related clinical area is required.
- Previous inpatient or outpatient hospital experience is preferred.
- Current licensure is preferred.
- A positive attitude and desire to help patients improve their lives.
