Social Worker (LSW) Per Diem, Shift/Hours Varies, Atlantic Health Chilton Medical Center, NJ

Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center.

Responsibilities

• Provide Social Work coverage to facilitate discharges.
• Meet with patients and families to assess and provide discharge planning support, resources, referrals, and transfers to Extended Care and inpatient psychiatric facilities.
• Work closely with the Healthcare Team to implement the plan of care.
• Obtain insurance authorizations as required.
• Communicate with community resources and area facilities.
• Perform phone, fax, and electronic documentation.

Requirements

• Minimum Education: MSW and NJ LSW.
• Minimum Experience: 1-year medical setting (acute care hospital) or 1-year externship (preferred).
