About the role
Provides social work services and discharge planning to patients and families in the healthcare setting to meet individual health and psychosocial needs in the promotion of wellness and cost-effective outcomes. This position also transitions patients to alternate levels of care and provides crisis intervention as needed.
Requirements
- Bachelor's degree in social work from an accredited program
- Current state social work licensure
Pay
$35,443.20 - $53,164.80
Schedule
0.6 - 0.9 FTE flex schedule; Possible remote/hybrid option; Shift rotation: FTE Flex Days (United States of America); Shift start/end: Day Shift - Flex; Weekends: included; Holidays: No call obligation; No union.
