About the Role

Responsible for the social assessment of all adolescent patients and intervention on behalf of any patient who requests or is referred for social services. Includes the mutual development and implementation of a plan designed to strengthen the life skills of individuals.

Responsibilities

• Assess client’s social and emotional needs and jointly, with the client, develop a plan to meet those needs.
• Provide concrete services to the client and family, or assist them in securing such services.
• Record pertinent data in the client’s medical record concerning services given.
• Provide short-term goal-oriented social work intervention; a minimum of six cases will be carried by each social worker for short-term counseling. Statistics are to be submitted monthly.
• Identify outside resources for referrals and establish a working relationship (network or contractual); monitor the effectiveness of contracted services and make necessary adjustments.
• Conduct, after appropriate training, HIV universal counseling, including pre- and post-test counseling, ongoing support, and case management where needed.
• Participate equally in the prenatal, family planning, and primary care programs, including initial psychosocial assessments of all patients, and ensure appropriate financial screening. For adolescents, conduct an in-depth assessment to include adjustment to pregnancy, familial and social supports, parenting skills, etc.
• Provide health education services, including individual and group sessions.
• Participate in staff development opportunities.
• Facilitate related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
• Serve on committees or task forces, as assigned.
• Co-facilitate group work services for special needs populations.
