About the role

As a Social Worker at DaVita, you’ll be a vital member of each patient’s core care team. You will support and advocate for patients, helping them understand their rights and responsibilities while guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. This role allows you to build meaningful, long-term relationships with patients and their families in an intimate outpatient setting.

Responsibilities

• Support patients in understanding their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial challenges of End Stage Renal Disease.
• Advocate for patients within the dialysis care setting.

Requirements

• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) with a specialization in clinical practice.
• Current LMSW or LCSW license in the state of New York.
• Minimum of 1 year of experience post licensure.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.

Pay

The salary range for this role is $80,000 - $125,000 per year. Social Worker Intermediate roles are compensated between $86,000 - $93,000 per year. Compensation depends on qualifications, skills, competencies, and experience. Location-specific minimum wage details apply as follows:

• New York Exempt: New York City and Long Island: $66,300/year; Nassau, Suffolk, and Westchester counties: $66,300/year; remainder of New York state: $62,353.20/year.
• New York Non-exempt: New York City and Long Island: $17.00/hour; Nassau, Suffolk, and Westchester counties: $17.00/hour; remainder of New York state: $16.00/hour.
