Responsibilities

  • Completes patient assessment, reassessment and treatment plans as clinically indicated.
  • Absess for risk and safety factors and be able to recommend next steps in treatment.
  • Provides clinical interventions which may include but not limited to counseling, individual sessions, groups, bereavement and crisis intervention as well as assessment and intervention in cases involving all family/patient dynamics.
  • Completes the required documentation and paperwork as it relates to compliance and other regulatory guidelines.
  • Works collaboratively with colleagues, fostering positive relationships within and beyond the team.
  • Approaches challenges constructively and professionally, and partners with psychiatric staff and other health professionals to ensure high-quality diagnosis and treatment.
  • Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral.
  • Identifies potential barriers, limitations & lack of coping abilities, adjustments, functional status.
  • Maintains knowledge of available community resources, evaluates for abuse concerns across populations, and ensures timely referrals.
  • Maintains, utilizes and applies evidence-based therapeutic practices, and treatment modalities.
  • Contacts and communicates with insurers, including managed care companies to clinically justify treatment to negotiate approval for clinical services.
  • Attendance of required department, patient and family meetings.
  • Maintains and Models Organizations Health Values.
  • Demonstrates regular, reliable and predictable attendance.
  • Performs other duties as required.

Qualifications

  • Licensed Master Social Worker (LMSW), Licensed Professional Counselor (LPC) or Associate (LPCA).
  • Requires at least one year of demonstrated clinical background and hospital or crisis experience; relevant internship experience may be considered.
  • Preferred: Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
  • Required to submit licensure application materials for LCSW or LPC within two years of hire and successfully pass the examination within six months of eligibility.
  • Requires at least 3 years of experience as an LCSW.
  • Must have prior experience leading, supervising, or coordinating the work of clinical or support staff.
