Job Description The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a full time Social Worker to join our team! Job Responsibilities Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program. Coordinate and facilitate care planning and completion of all documentationFacilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends. Skills & Requirements Job Qualifications: MSW from an accredited School of Social Work. Valid New York State License in Social Work. (LMSW)Five (5) years dementia care experience Bi-lingual (English/Spanish) a plusCertified in Dementia Care a plusExperience in adult day care center preferable.Solid written, communication and organizational skills
