Job Title: FULL TIME (LMSW) SOCIAL WORKER Hourly Rate: $31 - $42 Location: 1200 Waters Place, Bronx, NY 10461 - Onsite Position Summary The Licensed Master Social Worker (LMSW) provides professional social work services to individuals, families, and groups to support their emotional, social, behavioral, and mental health needs. The LMSW conducts assessments, develops service plans, provides counseling and crisis intervention, coordinates community resources, and collaborates with multidisciplinary teams to ensure clients receive appropriate care and support. The LMSW advocates for clients, maintains accurate documentation, and promotes overall well-being while adhering to ethical, legal, and professional standards. Responsibilities Schedule /Shift All shifts available, with the greatest need on weekends (Friday through Sunday)Per diem shifts only Key Duties Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program. Coordinate and facilitate care planning and completion of all documentationFacilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends. Requirements: Education MSW from an accredited School of Social Work. Licenses/Certifications Valid New York State License in Social Work.
