About the role

DaVita is looking for a Licensed Social Worker (LMSW) to support patients and their families at our Minneapolis and New Hope Dialysis Units in the Minneapolis, MN area. As part of our team, you’ll help patients navigate the physical, mental, emotional, and financial demands of End Stage Renal Disease while building meaningful relationships in an intimate outpatient setting.

You’ll advocate for patients, guide them in understanding their rights and responsibilities, and work in a positive environment where your growth and work-life balance are prioritized.

Responsibilities

• Support and advocate for patients adjusting to life on dialysis.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial challenges of End Stage Renal Disease.
• Build long-term relationships with patients and their families in an outpatient setting.
• Collaborate with a team that values mutual support and positivity.

Requirements

• Licensed or registered in the state of practice as required by state regulations (LMSW or LCSW).
• Meets all state required regulations to practice in the dialysis setting.
• Master’s degree in Social Work (MSW) with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, with functional proficiency in DaVita-specific applications within 60 days.

Pay

The salary range for this role is $61,000 – $97,000 per year, depending on qualifications, skills, competencies, and experience.

Schedule

• Full-time, Monday – Friday.
• Covers both the Minneapolis Dialysis Unit (Minneapolis, MN) and New Hope Dialysis Clinic (New Hope, MN).
